The result gave Real a 4-2 win on aggregate, following their 3-nil win in the opening leg.
The Spanish side will play Italian club Juventus in the June 4 final in Cardiff.
Atletico roared into a two-goal lead with a Saul Niguez header and Antoine Griezmann's penalty, but Real scored in the 42nd minute to dampen their hopes of a comeback.
Real have now eliminated Atletico from the competition in four consecutive seasons, including the dramatic 2014 and 2016 finals.
Zinedine Zidane's side can become the first team to retain the Champions League, though AC Milan won back to back titles when it was the European Cup in 1990.
