The Ministry of Health has taken further action against the Belhaven rest home and its nurse manager.
The premises in Epsom was shut last week. A former employee faces an assault charge, after an incident in which an elderly resident had her mouth taped shut.
The ministry has now followed up on the closure by issuing a cessation order, to prevent Belhaven Limited trading as a rest home.
This will stop the business from providing any health services, at any location, from Friday.
It has served a separate order on the nurse manager, meaning she cannot practice as a nurse or caregiver, or be involved in running any health related organization.
The woman is a registered nurse, who has also been reported to the Nursing Council.
The Ministry says it has been forced to do so in the interest of public safety.
Quality and safety manager Rose Wall says there were overwhelming concerns about Belhaven.
The closure takes effect from Friday. Only two residents remain and Ms Wall says the ministry will help relocate them.
