9:41 pm on 7 November 2014

Police have confirmed a body found in a west Auckland stream is that of a man who ran away from Waitakere Hospital.

This afternoon police identified him as Adam Tan, who went missing nearly two weeks ago.

Waitakere Hospital general manager of mental health Ian McKenzie said the 22-year-old had agreed to go to a mental health unit, but sprinted off on the way there.

Dr McKenzie said there had been no indication that he would do that and a preliminary investigation found staff correctly followed procedures.

He said Waitemata district health board extended its condolences to Mr Tan's family and friends.

The death would be referred to the coroner.

A member of the public found the body in a stream behind the Trust Stadium in Henderson on Monday morning.
