An inquest has opened into the death of a woman in a rest home in Tauranga in February 2010.
Lillian Wilson, 87, died after a hoist being used to transfer her from her bed into a chair, gave way and she fell and hit her head.
Mrs Wilson fell about one metre and received serious head injuries.
She never regained consciousness and died in hospital two days later.
Mrs Wilson had been in the Cedar Manor Home in Tauranga for seven years.
