The US Occupational Safety and Health Administration has issued five violations to a store in American Samoa following the deaths of three men in January this year.
The men died when a crane touched overhead power lines while lifting beams at a construction site at the Industrial Park Tafuna.
The men were employed by Manua's Incorporated.
In a report, the administration issued five violations on June 19 and four of them were labelled "serious" with a proposed fine of $US8,873 for each serious violation.
The fifth violation was labelled "other" with no fine imposed.
The current penalty or fine for the serious violations total $US35,492 and was contested July 10 by Manua's Inc.
