Five of the country's public radiology departments in hospitals are at high risk, and another two are at medium risk.
A further three are not even trying to comply with international standards. The 10 departments make up around a third of all public radiology.
This has come to light in a new one-page report released to RNZ by the inspection body International Accreditation NZ Scanning is carrying on.
The risks include "aging equipment", "insufficient personnel" and "radiology information system concerns".
Reporter Phil Pennington obtained the report.
Te Whatu Ora declined to come on Morning Report to answer questions about the state of radiology departments.
