An inquest has heard social workers believed a violent offender was a danger to them, but approved him to live with a woman and her young children anyway.
A few days after this approval, the woman's toddler son was found dead at his Southland home, in late 2015.
The coroner has already ruled the man probably intentionally inflicted the injuries.
