On Tuesday, we reported that ACC mixed up the files of two people with similar names.
The medical notes of one of them were attached to the file of a person we called Sarah, and those notes were sent to Sarah.
RNZ reported that the notes were then used to in assessing how much Sarah received in compensation.
However ACC says while the files were mixed up and another client's notes were sent to Sarah, those notes did not determine what compensation Sarah received.
RNZ accepts we got that wrong and apologises for the error.
