The Solicitor General has ordered an inquiry into the death of a woman in Waikato Hospital 11 years ago.
Seventy-eight-year old Carmen Walker bled to death just hours after a cancer operation in 2010.
The post mortem at the time concluded that her heart was not able to pump enough blood.
Now the pathologist who gave that diagnosis says he wasn't told about massive blood loss during her surgery and he's changed his mind.
