Complaints of physical and verbal abuse, neglect and intimidation of residents and staff are being investigated at a Lower Hutt rest home.
The District Health Board is looking into five complaints at the Woburn Masonic Care village, where one unidentified staff member wrote a warning that 'someone will die needlessly' if neglect continues.
