A review has found Hawke's Bay DHB ignored its own staff before accidentally cancelling home help for hundreds of elderly and disabled people.
In January, the DHB sent a letter to 600 people cancelling their house-work service.
It reinstated the service a week later and apologised to those who were angered by the decision.
Now the DHB has published an internal review of what went wrong.
