WorkSafe CEO on investigations report

An independent report on workplace health and safety regulator Worksafe has found a number of failures in the way investigations of deaths and injuries on the job are handle.

Yet the report draws the conclusion that investigations are generally of good quality In an internal report, Worksafe's own response is that its methods are outdated and rely on old technology.

Dion Neems, whose 6-year-old daughter was run over by a rubbish truck three years ago, said that the report did little to address his family's grievances with the regulator.
