Gisborne father says report on WorkSafe investigations hard to take

State regulator WorkSafe admits in internal documents investigations into deaths and injuries have used an outdated approach.

The documents released to RNZ under the OIA, also say the lack of technology back up is ratcheting up pressures from ever-more complex investigations.

A review of five cases says on the whole investigations are good quality Yet it goes to describe multiple shortcomings such as a lack of staff, training, supervision, record-keeping and understanding of evidence.

WorkSafe's response shows it has not coped since law changes in 2016 gave it more powers.

It is now making changes.
