Children's Commissioner alarmed by toddler's death

The Children's Commissioner is deeply disturbed by the case of a toddler who died from being physically abused after a hospital failed to notice his injuries. Despite the 16-month-old being taken four times to hospital, staff failed to spot the boy's fractured leg or other injuries including bruising, a broken tooth, and blackened fingernails. The toddler then died ten to 12 days after being discharged from hospital. The Health and Disability Commissioner Anthony Hill has recommended the health board front up to a tribunal - the most severe recommendation he can make.
