A fatal car crash has sparked a slew of unanswered questions about the safety of car warrants of fitness. This after the Transport Agency stood by and let a Dargaville garage keep on issuing warrants despite years of warnings it wasn't checking vehicles properly. This only came to a stop when the garage gave a warrant to a car with a frayed seatbelt - it crashed and the passenger in the front seat died. Phil Pennington is looking into this.
