Auckland Council has launched an independent review into health and safety at its ports following the death of a stevedore on a night shift.
Pala'amo Kalati was fatally injured while working on a ship at Fergusson Container Terminal a fortnight ago.
The death is also being investigated by Maritime New Zealand.
