The Chief Ombudsman is calling for a complete overhaul of Oranga Tamariki, saying it is failing on almost every level.
Judge Peter Boshier has found the agency is still regularly failing to follow its own laws in a way he says is totally unacceptable.
But an advocate for children in care doesn't trust that Oranga Tamariki will heed the recommendations made.
