Oranga Tamariki was warned about harmful restraints at a Christchurch Care and residence facility months before it was finally closed.
The agency announced Te Oranga would close in July - following an investigation by Newsroom revealing footage of staff tackling a young person to the ground.
RNZ has obtained a monitoring report by the Office of the Children's Commissioner into Te Oranga published in December last year.
